Sub-Acute Care

The Sub-Acute Care Program assists patients with medically complex conditions who cannot return to the community safely after discharge from an acute hospital setting.

Services are designed to assist with increasing functional status while
training caregivers to prepare for a smooth discharge.
 

Care Team

Providing compassionate and comprehensive care is the primary focus of our experienced team of professionals. Using an interdisciplinary team approach, the professional staff designs an individualized care program to address a patient's physical, psychological, and social and spiritual needs.

The team includes:

  • Internal medicine and geriatric medicine physicians
  • Nursing staff
  • Physical therapists
  • Occupational therapists
  • Respiratory therapists
  • Speech-language pathologists
  • Social workers
  • Therapeutic recreation specialists
  • Dietitians
  • Psychologists
     

Admission Criteria

  • 18 years and older
  • Require intensive nursing services each day
  • Require specialized medical care
  • Have conditions that do not require acute hospitalization
  • Have the potential to return to a community setting
     

Treatment Plans

  • Intravenous therapy
  • Medication adjustment
  • Respiratory therapy
  • Post-surgical care
  • Rehabilitation therapy
  • Wound care
     

Amenities

  • Day rooms where patients can enjoy watching television, reading or visiting with family and friends
  • Free local telephone service
  • Numerous planned activities on and off the units
  • Meals served at the bedside or in the dining room
