Lay Health Advocate Program

Lay Health Advocate Program participantsThe Lay Health Advocate Program is an innovative way in which the Healthy Community Partnership enables Johns Hopkins Bayview Medical Center to reach beyond its walls to support vulnerable patients with chronic illnesses.

In this program, Medical Center professionals provide community volunteers with the training and ongoing support they need to work one-to-one with individuals who require assistance managing chronic illnesses and medical conditions. These lay health advocates help patients prepare for their medical appointments, updating and organizing the information their physicians will need. After the appointment, the advocates assist with follow-up care, helping patients monitor their conditions and follow treatment recommendations. This can include helping patients organize their medications and schedule additional appointments. When appropriate and requested by patients, advocates may even accompany patients to their medical appointments.

Lay health advocates also talk with patients about taking steps to prevent additional medical issues, such as injuries resulting from falls, and how they can maintain control over their medical care even if a serious illness or injury were to prevent them from making a decision or communicating their wishes (advance directives). Advocates also provide emotional support and reassurance for patients.
