Pediatric and Adolescent Gynecology

Pediatric and Adolescent GynecologyLocated in the Children’s Medical Practice and OB/GYN clinic of the Johns Hopkins Bayview Medical Offices, the Pediatric and Adolescent Gynecology Program provides comprehensive gynecological care to patients from birth to age 21.

The program manages a broad spectrum of gynecological problems facing the female pediatric and adolescent patient with an emphasis on the unique needs of her age group. Some of the problems encountered in these patients are:

  • congenital malformations
  • disorders of puberty and menses
  • vulvovaginitis
  • ovarian pathology
  • pelvic pain
  • vulvar skin conditions
  • urethral prolapse and trauma
      

Congenital malformations include, but are not limited to, mullerian anomalies, androgen insensitivity and an imperforate hymen. Mullerian agenesis is managed by a comprehensive program that includes gynecological treatment, psychiatric care and a support system for this challenging diagnosis. Endocrine disorders including precocious puberty and primary amenorrhea are treated. The program also offers care of the developmentally delayed patient.

Evaluation generally includes a brief office visit with the child and one or both parents to obtain a history of the problem. When a gynecological exam is needed, a specially trained physician performs the exam in a manner that is not traumatic to the child. Depending on the problem, additional testing may include an ultrasound, MRI or blood test.

If a more in-depth exam is needed, an exam under anesthesia may be required. When surgery is necessary, surgical techniques to optimize safety and outcomes in younger patients are utilized. Experts within the program have worked together to develop several novel surgical techniques to improve outcomes in congenital anomaly reconstruction and neovagina surgery.

For complex problems, a multidisciplinary team may be used, including specialists in adolescent medicine, pediatrics, pediatric hematology, endocrine, plastic surgery, psychiatry, urogynecology and urology.
