Acute Rehabilitation

Acute care rehabilitation is provided to many patients while in the hospital. Services may consist of any combination of occupational therapy (OT), physical therapy (PT) and speech-language pathology (SLP).

Our team of therapists works with physicians, nurses, case managers and the rest of the interdisciplinary team to coordinate a safe discharge plan. Patients and caregivers participate in developing a treatment plan addressing the patient's needs from a functional perspective. Rehabilitation assessments can help identify deficits in the areas of mobility, activities of daily living, cognitive skills, speech and swallowing.

Treatment can provide compensatory techniques to help bridge the gap in ability allowing the highest level of functioning possible. Assistive devices may be provided to enhance mobility and independence in self-care and memory. Patient and family education is a key component to transition to the next level of care.

Patients are referred for therapy services by physician determination of need.
