Make An Appointment
- Call or schedule an appointment with your primary care physician to write a referral/consult for physical therapy, occupational therapy or speech therapy.
- Some specialty clinics (orthopedics, neurology) may be able to write a referral/consult without seeing your primary care physician depending on your insurance.
- Fax the referral to 410-550-1390 or hand deliver to the outpatient rehab department.
- Once the referral is submitted, the referral coordinator in our department will contact the patient’s insurance company to get authorization or verify benefits.
- Once the authorization is approved and/or benefits are verified the patient will then be contacted by phone to schedule an appointment.
- If you any questions or need more information about the process, please call us at 410-550-0414.
