OT Student Information

Welcome to the Occupational Therapy Department at the Johns Hopkins Bayview Medical Center. We look forward to your clinical affiliation. We contract with various schools throughout the country.

This site contains the necessary information you will need in order to begin your fieldwork experience, including the required paperwork you’ll need to complete, information about the Rehabilitation Department, parking fees and contact information. Please note that housing options are available upon request; however the hospital does not endorse any housing facility. This information is provided for informational purposes only.

Below is a list of the required paperwork that should be submitted at least two weeks in advance of the start of your internship. Please fax to Adersh O'Day or Ginny Carman at 410-550-1390. After faxing, please e-mail to confirm that your fax has been received: Adersh O’Day, student coordinator, at aoday@jhmi.edu.

1. Copy of a current CPR certification (within two years)
2. Copy of Mumps, Measles, and Rubella shots
3. Copy of current PPD/TB testing (within one year of internship)
4. Completed Background Authorization Form (please also read a Summary Of Your Rights and the FCRA Notice)

Please sign and bring the following forms on your first day:

2. MediLinks User ID Request Form
