How to Apply

To apply for the sleep medicine fellowship program, please submit the following:

1. Completed Sleep Medicine Fellowship Application (PDF)
2. Current curricula vitae (CV)
3. Personal statement about what attracted you to sleep medicine (less than 500 words)
4. Three letters of recommendation (including one from your program director)

All applicant materials are reviewed, but only a few applicants are selected for an interview. Interviews usually are scheduled during the months of August and September, with the intention of being able to notify those that where interviewed of the final decision by the first week of October.

Applications for the July 2011 fellowship will be accepted January 1 through July 30, 2010.
